Inquest Verdict
During the afternoon on 27th July 2008 in the shower room on Halswell ward in the Shaftesbury Clinic, Neil Stanhope Hanged himself, whilst the balance of his mind was disturbed. It is concluded the death was accidental, various circumstances contributed to Mr Stanhope death, namely:
1) The Key Worker failing to adequately communicate with Mr Stanhope, properly complete his careplan or provide adequate reports for the ward round meetings.
2) Insufficient communication between the members of the nursing staff (be it permanent or agency) and with other members of MDT including written communications concerning Mr Stanhope.
3) Inadequate supervision and training of the nursing staff and healthcare assitants including their conduct of observations and key worker record keeping.
4) The failure to properly communicate incidents recorded on 22nd June 2008 and 6th July 2008 prior to the decision being made by the MDT to reduce Mr Stanhopes observation from 1:1 to 5 minute observations whilst his medication was being reduced.
5) Inadequate 5 minute observations of Mr Stanhope on 27th July 2008.
6) Inadequate staffing levels on Halswell ward on the 27th July 2008.
7) The presence of potential ligature points in private areas such as the hsower room on Halswell ward.
8) Managements failure prior to 27th July 2008 to address staffing concerns that staff members reported to management.
Although the facts below did not contribute or cause Mr Stanhopes death, the following factors were relevant to the circumstances surrounding his death:
1) Inadequate maintenance of the door lock on the shower room door on Halswell Ward.
2) The failure to keep a green emergency bag on Halswell Ward.
3) The unexceptable manor in which the five minute and 1:1 observation records were completed on 27th July 2008 on Halswell Ward.

Hi my name is Ben Horsburgh, i went to secondary school with neil, i have been living in scotland for the last 5 years and only just found out about what happened to Neil, just wanted to pay my respects to his family and friends, i was very close with Neil at school and he always made me laugh, i was only telling my wife last week about one of the funny things he had done at school, i am absolutlely gutted and can't believe another angel as be taken from us, in my heart where i will keep the memories we shared, but the sun never goes down on a legend, RIP my friend, never forget you brother, we will meet again, you can count upon on, i would have never survived secondary school without you Neil, miss you bruv x

god is so forgiving and I am truly free,
and i will wait for you to come and share my life with me,
so when tomorrow starts without me, don’t think we're far apart,
for every time you think of me
